CCIT Mentorship Application

Thank you for your interest in participating in the CCIT Mentorship Program.

The program is offered during both Spring and Fall semesters, beginning with an in-person orientation at the start of the term and concluding at the end of the semester. The information you provide on this form will help us match you with a mentor. Please note that participants must be at least 18 years of age.

While mentors and mentees may have general discussions about job placement, the primary purpose of this program is to provide career guidance and professional development.

I understand that, if selected as a Mentee, that I am expected to meet with my Mentor weekly during the program's timeframe, and be prepared to come to each meeting with goals, questions or other topics for discussion. I also understand that the purpose of this program is career guidance and skill development, and not job placement or academic tutoring.
