Joint Services Transcript (JST) - Request to Post Courses

Students enrolling after serving in the U.S. military are required to submit their Joint Services Transcripts (JST). The Office of Admissions & Records will evaluate the JST and enter course equivalencies in Peoplesoft. However, courses will not be posted to the student's record unless requested by the Career & Academic Advisor.

This survey should be completed AFTER you have met with the student, reviewed their record and determined it is in the student's best interest to have the courses posted. Please reference the Florida Department of Education's Military Course Equivalency List when reviewing the student's transcript.

Once credits are posted to the student's record, they cannot be removed

This survey is for use by Career & Academic Advisors only. It should not be submitted by students.

By submitting this survey, I am certifying that I have reviewed the JST course equivalencies published by the FLDOE with the student and we have agreed it is in the student's best interest to post the courses to their record. The student understands that (1) all courses will be posted and (2) once posted, the courses cannot be removed from their academic transcript.
