UCF Transfer Connect Student Survey

Please fill out the fields below and submit to Sheryl Mundorff so the student can be added to the UCF student group and service indicator.

This survey should be used for the following: 
• Referring new UCF students
• UCF students changing to Major outside of the UCF Program (SPC major or other university)

If it is a major change, make sure to write in the notes section of the survey the major the student is changing to outside the UCF Program and the reason they are choosing to leave the program.  If sending students for future follow up please indicate this in the notes along with details, for example: waiting on HS transcripts before I can update program plan page
