MIRA Questionnaire

The MIRA questionnaire is structured to provide us with the information needed to properly place you within the music program.  It does not function to exclude any prospective student from MIRA.  For that reason, we ask that you be as complete and detailed as possible in your answers. 

Once completed, you will receive an email that recommends specific coursework.  Once you have received the email, please bring a copy to HS 118 for registration.  This form does not register a student for any courses.
